Fillable Forms Questions
Do I have to be an ACORD Advantage member to use ACORD Forms?
In order to make use of the ACORD Forms available from this website, yes.
However, ACORD Forms are made available to the insurance industry through a variety of options. There are many solution providers, insurance carriers and reinsurers licensed to include ACORD Forms in their products, services and/or websites. You do not have to be an ACORD Advantage member to use ACORD forms provided by a licensed partner.

I clicked on a form and an "Access Denied" page appeared. What do I do?
If you are an Advantage Member, Sign In (the sign in fields are located on the left hand side of your browser window) to gain access to that form. If you are not an Advantage Member, you will need to join the Advantage Program. Please note that if your agency's location is already a member of the Advantage Program, you will still need to enroll with the Advantage website, but do not need to pay the membership fee.
If you are certain that you are signed in, it may be that your membership to ACORD Advantage has expired. Please contact memberservices@acord.org, or call 800-444-3341 ext. 506.

I clicked on a form but nothing happened. What do I do?
For your workflow convenience, ACORD provides several different versions of the ACORD forms. Each version requires that you have the associated program installed on your machine.
- Using ACORD Fillable Forms - IBM Format requires an installation of the ACORD Viewer.
- ACORD PDF and ACORD Fillable Forms - Adobe PDF files require the free Adobe Reader.
If you are certain that you have the right software and are signed in, it may be that your membership to ACORD Advantage has expired. Please contact memberservices@acord.org, or call 800-444-3341 ext. 506.

I emailed a form to my customer/trading partner and they were not able to open it. Why?
To view an ACORD Form, you need to have the associated software installed. If you emailed an ACORD Fillable Form - IBM Format, the recipient will need to install the ACORD Viewer. If you emailed the ACORD Fillable Form - Adobe PDF version of the form, the recipient will need to have the Adobe Reader installed. Please be aware that viewing Adobe PDF files will require users to have licensed versions of those products on their machines.

I got my forms from another website. Why should I use the ACORD Advantage Forms?
ACORD is the originator of all the ACORD Forms as well as updated ACORD Forms. If you need to use a form, and it is not available from your other source or is out of date, you may secure the most current form through ACORD Advantage membership.

Do I need to join the ACORD Advantage Program to receive Forms Notifications from ACORD?
No, the Forms Notification Service is complimentary. If you already recieve our forms notifications and wish to always be aware of the current forms, you should continue to receive it. If you are not recieving our forms notifications and would like to recieve it, you can register to this website. This will ensure that you receive the complimentary Forms Notification Service. I just want to place an order for paper forms. How do I do that?
Paid members of the ACORD Advantage Program can place orders for P&C paper forms with the ACORD Member Services department. Please call in your request to 800-444-3341 ext. 506, fax it to 845-620-3600, or email memberservices@acord.org, Monday through Friday, 8:30 am to 4:30 pm.

What are ACORD Fillable Forms?
ACORD's fillable forms have the same look and feel as the paper and PDF versions you may be used to, but have the following new time-saving features and capabilities:
- Fill, Save and Send - You can fill out each form on screen and with the click of a button, email, print, save or fax it!
- On-Screen Help - Every field features on-screen help and prompts to make filling out the forms easier.
- Forms Instruction Guide - An “Instructions” link brings up the full instruction guide for that form, right on screen.

I can’t find the ACORD Fillable Forms. Where are they located?
All available versions of the ACORD Forms are provided through our Forms Search. Advantage Members can simply sign in and search for the form they are interested in. The ACORD Fillable Forms - IBM Format are found in the "ACORD Fillable IBM" column with available forms represented by the symbol "A" in blue. If there is no "A" symbol, then that particular form is currently not available in the fillable format. The ACORD Fillable Forms - Adobe PDF are found in the "ACORD Fillable Adobe" column with available forms represented by the PDF symbol and "Fillable" below it. If there is no symbol, then that particular form is currently not available in the fillable format.

I noticed there are various columns on the forms search results: ACORD Fillable IBM, ACORD PDF, and ACORD Fillable Adobe. What are these and which should I choose?
All available versions of the ACORD Forms are provided through our Forms Search. Advantage members have their choice of the version that fits their workflow best. There currently are 3 formats: the “ACORD Fillable IBM” format which requires the download of the free ACORD Viewer; the “ACORD PDF,” Adobe's Portable Document Format, which allows you to print out a blank form and fill it out by hand or typewriter; and the "ACORD Fillable Adobe" which requires the download of the free Adobe Reader to view, fill and print the form and the purchase of Adobe Acrobat to save and email the form.

What must I do in order to have access to the ACORD Fillable Forms?
You must participate in an ACORD program that provides access to the forms, such as the Advantage Program.

What are the ACORD Viewer's system requirements?
Your computer must be running any one of these operating systems:
- Windows 98, ME or XP
- Windows NT, 2000*
And one of the following browsers:
- Microsoft Internet Explorer version 4.0 or higher
- Netscape Navigator version 4.07 or higher
Regardless of which browser you use, support for JavaScript and Java applets must be enabled. * Note: If you are using
Windows NT or Windows 2000, your user account
must have sufficient privileges to install software
and modify the computer's configuration. Therefore,
your account must be a member of the local administrator
group or the power user group.

I have an Apple/Macintosh. Can I use ACORD Fillable Forms - IBM Format and/or the ACORD Fillable Forms - Adobe PDF?
IBM Format: No. Currently the ACORD Viewer only works on the following PC operating systems:
- Windows 98, ME or XP
- Windows NT, 2000*
Adobe PDF: Yes. The Adobe Reader and Adobe Acrobat works on Apple/Macintosh systems.

I am unable to save or send my ACORD Fillable Form - Adobe PDF format. Why?
Purchase and installation on your computer of Adobe Acrobat is required if you want to save or send an Adobe PDF format fillable form. The free Adobe Reader allows users to view, fill and print but not save or send the form. For more information, click here.

Can I download a fillable form and its contents to my Agency Management System?
You may download and save ACORD Fillable Forms onto your PC or laptop for use offline. However, the functionality to download or export the information on the form into your agency management system is not currently available.

Where are the Forms Instructions on the ACORD Fillable Forms?
If you are using the ACORD Fillable Forms in the IBM or the Adobe PDF formats, there are instructions automatically provided when you scroll your cursor over each field.
In the IBM format, if you would like to secure a summary of all the field instructions, you may click on the “Instructions” choice on the Fillable Forms menu located at the top of that form.
In the Adobe PDF format, if you would like to secure a summary of all the field instructions, you must go to the Search Page and click on the "FIG" icon on the ACORD Advantage Search Results screen.
